We don't know that for sure. He could be the Valeyard, the 13th regeneration from Baker's era who tried to erase all of the Doctors from 6 & 7 on to keep himself alive

This room doesn't have as strong an effect on the doctor because he doesn't have a "deepest" fear so the hotel just picked a recent one he had instead. The door doesn't say "11" because the doctor is inside, it says it so he knows that it is his door.

My theory is that he saw Amy "with" Rory in his TARDIS, that's why he says "who else", you can hear the TARDIS, and he puts a "do not disturb" sign on the door for them.
